Terms and Conditions:

For patients whose prescriptions for Flarex are covered by commercial insurance, use of this card may reduce your copayment responsibility to as little as $35.

For patients whose prescriptions for Flarex are not covered by either commercial or government insurance, use of this card may reduce your cost for Flarex to as little as $55.

This program is not valid for patients with prescription coverage under Medicaid, Tricare, VA, or DOD.

This program is subject to overall maximum support amounts and is valid for up to 4 prescriptions.

Patients participating in Medicare Part D or a Medicare Advantage prescription drug plan who are eligible to use the Flarex Copay Card Program must agree to the following conditions. Patient must agree to not seek reimbursement from their Medicare or Medicare Advantage prescription plan for their out-of-pocket costs for Flarex purchased with the card.
