  • Is the person who will use the coupon younger than 65 years of age?

  • Does the person who will use this coupon have health insurance that covers prescription drugs?

  • Please enter the 5-digit ZIP code of the person who will use this coupon.

  • Re-enter the 5-digit ZIP code of the person who will use this coupon.
